HOW TO AVAIL OF THE SERVICES:

*For Medical Certificate and Releasing of patient's records, a written order from the attending physician is required.

*For Birth Certificate, one has to fill-up the Baby's data form and submit it to the office for processing.

*For Death Certificate/Fetal Death Certificate, a data filled-up by the family member is required.

It is facilitated by the nurse on duty.
