Patient Guidelines
PHARMACY DISCOUNTS
IMPORTANT NOTE:
If the member is applicable for two or more discounts, only the highest discount will be given.
Patient Discounts:

Discounts are offered to the following:

GSIS members - 30% of the price mark-up

GSIS beneficiaries (children of the member under 21 years, old, parents of the member at the age of 60 y.o., and spouse of the member) - 10% of the price mark-up

PWD members – 20% without VAT

Senior Citizens – 20% without VAT

Cash Discount for admitted patients

GSK discount card holders

Pfizer Sulit Card holders
Please present the following during each transaction to avail of the discounts:
1. Complete prescription
2. ID (for GSIS, PWD and Senior Citizens)
3. Purchase booklet (for PWD or Senior Citizens)
4. Discount card (if applicable)
GENERAL GUIDELINES IN DISPENSING REGULATED AND DANGEROUS DRUGS
I. Regulated Drugs (such as Alprazolam, Midazolam, Diazepam, Phenobarbital tablets)

Triplicate regular prescription – should contain your doctor’s S2 license

Doctor – should be affiliated with Perpetual Succour Hospital

Validity of prescription – two months from the date prescribed

II. Dangerous Drugs (such as Morphine products, Fentanyl, Midazolam and Diazepam ampules)

Triplicate yellow prescription – should contain your doctor’s S2 license
-
Diagnosis – must be written on the prescription

Doctor – should be affiliated with Perpetual Succour Hospital

Validity of prescription – two months from the date prescribed
