How Admission Works

Medical Assessment

Patients are screened and examined by the Resident Physician in the Triage. Those who need further evaluation and for probable admission are transferred to the Emergency Room except those DTR admission.

Arrival & Registration

Admitting order is needed to confirm admission of the patient

Room Assignment

Admission staff explain the estimated hospital costs to the patient or relative and request to correctly fill up the Patient’s Record Sheet and affix signature at the Consent for Hospital Care And Diagnostic Procedures form and Patient Orientation and Agreement form, after being thoroughly explained to by the admission staff.

Active Care & Treatment

Administrative Staff receives color-coded admission slip from the patient or significant other

Discharge & Billing Settlement

Patients or significant other are asked to give Partial payment to the cashier to complete the admission

Hospital Policy

→ Children Under 10: Signed waiver from parent/legal guardian required for hospital admission; parents must review and consent to medical treatments and procedures.

→ Restricted Hours: Visiting hours enforced from 9:00 PM to 9:00 AM with strict access control.

→ One Watcher Rule: Only one designated watcher permitted per patient during restricted hours; additional visitors must be politely but firmly turned away.

→ Authorized Exceptions: One watcher rule may be waived for critical illness, end-of-life care, pediatric patients, labor/delivery support, patients requiring cognitive support (with medical authorization), or when a doctor authorizes additional watchers due to medical necessity.

→ Access Control & ID Verification: All individuals must present identification at entry/exit points during restricted hours; unidentified watchers directed to nursing station for verification.

→ Visitor Documentation: Maintain detailed visitor logs including date, time, room number, patient name, visitor name, ID type, and signature; incident reports required for security breaches.

→ Communication & Environment: Maintain professional communication with visitors and staff; promptly address noise disturbances; report concerns to nursing supervisor.
