The Medical Records Department is dedicated to managing and safeguarding every patient’s health information with the highest standards of accuracy, confidentiality, and integrity. As an essential part of the healthcare team, we ensure that medical records are properly maintained, securely stored, and made available to authorized individuals in accordance with hospital policies, legal requirements, and data privacy regulations.
Our department provides comprehensive medical records services, including the processing and release of medical records, medical certificates, clinical abstracts, and other authorized medical documents. We are also responsible for medical record coding, scanning, indexing, filing, archiving, and the continuous maintenance of complete and accurate patient information to support quality healthcare delivery.

Processes requests for certified copies of medical records from patients, authorized representatives, healthcare providers, insurance companies, government agencies, and other authorized entities in accordance with hospital policies and applicable laws.
Facilitates the processing and release of medical certificates upon receipt of the required authorization and supporting documents.
Prepares and releases clinical abstracts summarizing a patient’s hospitalization or treatment for authorized purposes.
Prepares and encodes the Certificate of Live Birth and Certificate of Death based on complete and authorized medical documentation, ensuring accuracy, completeness, and compliance with applicable government regulations and hospital policies prior to submission to the appropriate civil registration authorities.
Responds to authorized requests for verification of medical records and other official medical documents while maintaining strict confidentiality.
Guides patients, relatives, and authorized representatives regarding documentary requirements, processing procedures, and turnaround times for medical record requests.
